First Visit Experience For Periodontal Disease

When you come in for your appointment please remember to bring:

  • Recent dental x-rays
  • Your referral letter (you can self-refer)
  • Complete list of medications currently being taken.

If your dentist has not given you a letter, please inform him of your appointment and ask them to send one to us either electronically or by post. Digital x-rays are also welcome.

During your visit to Radny & Valizadeh Periodontics and Dental Implant Centre, your Periodontist will:

  • Take a thorough medical and dental history.

  • Perform a comprehensive evaluation of your teeth, gums and review any x-rays to determine the extent and pattern of the disease and any bone loss.
  • A comprehensive report will then be sent to the referring dentist.
  • An itemised treatment plan will be established

Once the assessment has been completed, your Periodontist will discuss all treatment options available to you and then recommend a personal treatment plan to tackle the problem.

Please note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

During your visit to Radny & Valizadeh Periodontics and Dental Implant Centre, your Periodontist will:

  • Take a thorough medical and dental history.
  • Perform a comprehensive evaluation of your teeth, gums and review any x-rays to determine if implant placement is an option for you.
  • An itemised treatment plan will be established.

Once the assessment has been completed your periodontist will discuss the treatment options in full. An idea of costs involved will be discussed with you and a report sent to your dentist to inform them of treatment proposed.

Health Funds – HICAPS

We are one of a very few specialists participating as a HBF “Members Plus” Provider.  This can help reduce your out of pocket expenses significantly offering 60 – 90% rebate depending on your level of cover.

We accept ALL other major health funds and HICAPS is available at this Specialist Practice, which will enable us to process your health fund rebate on your behalf.

We are a private billing practice and fees are payable at the time of consultation. Fees are based on the Australian Dental Association (ADA) recommended fee level and vary according to time, procedures performed, content of consultation and treatment required.

We accept Cash, Cheque, EFTPOS, MasterCard, Visa and Amex.
